  • Format: Use Format A
  • Special Rules
    • Organization Name should be in Proper Case
    • If Organization Name (normally for departments, agencies, districts) is more than 2 words long, use acronyms instead
    • State Abbreviation should be 2 characters/letters and in UPPER CASE
    • For Counties, include the word "County" in the middle of the organization name and the state abbreviation
    • For other than counties, don't include "City", "City of", "Town", "Town of", "Township", "Village", "Village of"
